| Co-ordination of all rites & ceremonies, during and after they have been provided, including services and supplies provided by us, as well as those provided by third party suppliers, such as death notices and clergy. |

| Completion and filing of all documents necessary to carry out the services & supplies as requested, including, but not limited to death registration, burial permit, coroner's certificate for cremation, etc. You will also receive an unlimited supply of death certificates. |

| Transfer from place of death (hospital, private residence, retirement residence, nursing home, etc.) to Funeral Home, from Funeral Home to Crematorium and from Crematorium back to Funeral Home. This is based on a one attendant transfer (hospital). For transfers requiring 2 attendants (residences, nursing homes, etc.) an additional attendant of $100 will be required. |

| Embalming is a process of sanitation, restoration and temporary preservation of remains. Embalming is not legally required but is mandatory if there is to be an open casket present at the visitation. |

| Staff to coordinate and oversee two visitation periods at our premises for a period of up to 2 hours. If the visitation takes place off-site, a fee of $300. will be charged at a flat rate. Please enter in the quantity for the amount of visitation period you would like to have. |

| Staff to coordinate and oversee the ceremony (funeral service, memorial service, celebration of life, etc.) at the Funeral Home. If the ceremony is off site, the staff professional fee will be $725. |
